SBa_E0003: Consent Form

Description

The consent form is a document that should be written in simple, plain language to allow participants to fully understand what they are signing and agreeing to as part of the research being conducted.

It should cover/ state the following points:

  • The participant understands the information provided
  • The participant has been given a chance to ask questions regarding the research
  • The participant voluntarily agrees to take part in the project
  • The participant knows they are free to withdraw from the research at any particular point in time.
  • The participant has been informed about procedures regarding confidentiality.

It is important to remember to provide the participant with a copy of the consent form, and that the researcher retains the signed and dated original.
